A minimal field set
Adapt this list to the organization's approved purpose and legal advice. Do not add phone numbers, images, company details, signatures, or free-text notes by default if the building cannot explain why it needs them.

- Purpose or visitor category
- Recording staff initials or identifier

Make a paper template less exposed
Avoid asking visitors to write directly beneath previous visitors where names, destinations, and phone numbers remain visible. Use staff-operated sheets, page covers, individual slips, or another process that prevents casual disclosure.
Store completed pages in controlled access, define who may retrieve them, record copies or disclosures where appropriate, and dispose of them under the organization's retention policy.

Know when the template has reached its limit
A better paper layout can improve consistency, but it cannot provide instant filters, role-based remote retrieval, controlled CSV export, per-building form versions, or automatic staff attribution.
If management regularly searches old entries or operates multiple buildings, a digital logbook addresses the record-management problem rather than only redesigning the sheet.
